Optimal Treatment Strategies for Tuberculosis with Exogenous Reinfection

چکیده
Tuberculosis (TB) is a disease found in about one third of the world population. There is some controversy on the role of exogenous reinfection; with some epidemiologists supporting it while others consider it irrelevant. However, even at a minimal level, the presence of exogenous reinfection is capable of generating hysteresis and supporting multiple endemic states when the basic reproduction number (R0) is less than one. In this manuscript, we study the use of control in areas where TB is prevalent (R0 > 1) as well as in areas where some additional effort (R0 < 1) may be able to eliminate the disease. In this model, controls on social distancing, disease relapse, and treatment are incorporated to reduce the latently infected and actively infected populations, via the application of optimal control theory.

Exogenous reinfection as a cause of multidrug-resistant and extensively drug-resistant tuberculosis in rural South Africa.
BACKGROUND Multidrug-resistant (MDR) and extensively drug-resistant (XDR) tuberculosis (TB) are now major threats in areas of South Africa with a high prevalence of TB and human immunodeficiency virus (HIV) infection. The role of exogenous reinfection as a cause of MDR and XDR TB in these settings has not been determined. METHODS We reviewed data from patients with culture-positive TB who lat...

the risk of tuberculous infection is high [8, 9]. Our patient, and others described in previous reports [2-4], were in environments where they were heavily exposed to M. tuberculosis. In addition to varying degrees of immunosuppression, high levels of exposure to M. tuberculosis likely contributed to their risk of reinfection. The case described herein illustrates that diabetes may be a risk fa...
متن کاملImpact of HIV infection on the recurrence of tuberculosis in South India.
BACKGROUND There is limited information on the relative proportion of reactivation and reinfection at the time of recurrence among human immunodeficiency virus (HIV)-infected and HIV-uninfected patients who are successfully treated for tuberculosis infection in India. METHODS HIV-infected and HIV-uninfected patients with sputum culture-positive pulmonary tuberculosis were treated with short-c...
متن کاملOptimal control for a tuberculosis model with reinfection and post-exposure interventions.
We apply optimal control theory to a tuberculosis model given by a system of ordinary differential equations. Optimal control strategies are proposed to minimize the cost of interventions, considering reinfection and post-exposure interventions. They depend on the parameters of the model and reduce effectively the number of active infectious and persistent latent individuals. The time that the ...
متن کاملA model for tuberculosis with exogenous reinfection.
Following primary tuberculosis (TB) infection, only approximately 10% of individuals develop active T.B. Most people are assumed to mount an effective immune response to the initial infection that limits proliferation of the bacilli and leads to long-lasting partial immunity both to further infection and to reactivation of latent bacilli remaining from the original infection. Infected individua...
